In women with essential thrombocythemia, the absolute risk of a venous thromboembolism in the postpartum period is above a threshold where postpartum low-molecular-weight heparin prophylaxis should be considered. The absolute risk of venous thromboembolism in the antepartum period is not above a threshold where antepartum low-molecular-weight heparin is clearly indicated or below a threshold where low-molecular-weight heparin should be withheld. Thus, a preferences and values-based discussion is important when counseling women with essential thrombocythemia in pregnancy.
