ABSTRACT
The role of survivorship care in oncology is a critical one. National standards, guidelines, and recommendations add to the complex scope of this immense but vital discipline. What is the most practical approach to take to tackle the survivorship challenges faced by patients, caregivers, and medical practices? Minnesota Oncology’s survivorship team has come up with a tablet-based tool that they believe can help.
